Trinity Care Adult Day Care is a professional care setting in which older adults receive individualized therapeutic, social, and health services during the day when family members or caregivers have other commitments like jobs, school or other obligations. Our Adult Day Care Center provides a coordinated program of professional and compassionate services for adults in a community-based group setting. Services are designed to provide social and health related services to adults who need supervised care in a safe place outside the home during the day. Care in our facility is often an alternative to nursing home care, thereby allowing seniors to continue to live at home while providing respite care for the caregiving adult child or spouse. PACE CNY has been providing services since 1997. It is one of only 115 PACE providers in 32 states. PACE CNY, provides a convincing blueprint for the future of eldercare, where an interdisciplinary team approach to senior care can actually improve quality of life in a cost effective manner. The potential for improving quality of life is remarkable when you consider national PACE statistics, which mirror Loretto’s experience locally. A full 68 percent of older adults in the PACE program are 75 years and older. They have an average of 8 diagnosed medical conditions including ailments like hypertension, arthritis, diabetes, dementia (45 percent, with another 30 percent showing cognitive impairment) and coronary artery disease. At the heart of the PACE model is a team of geriatric healthcare specialists that provides in-depth evaluations and individualized care plans for each participant. Daily meetings allow the team to closely observe and manage all health and long-term care services for each individual. While nursing homes will always have a place in the total mix of services, families are often unaware of other options. Now, PACE CNY is your most flexible option! The Center for Medicare and Medicaid Services (CMS) and the New York State Department of Health (DOH) conduct bi-annual surveys to ensure a high quality of care.

PACE is a Program of All-inclusive Care for the Elderly. This is a long-term care program for older adults who wish to live at home but qualify for nursing home care. They need assistance in order to maintain their health, complete day-to-day activities and remain safe for as long as possible. As part of the program, all medical care is provided or coordinated by our team. Primary care services are provided by the physician at our health center. Other medical services may also be provided at the center, at the patient's home or at an off-site location. Transportation to and from medical appointments is included. In addition to medical care, we offer a day center, with recreational and social activities. This allows you to receive personal care and supervision during the day. As a LIFE member, you will access many of our services at the LIFE Center in Lackawanna, located on the campus of the OLV Senior Neighborhood. How often you visit the center and for how long depends on your personal circumstances and medical needs. Visits typically range from one to five days per week.

The Day Center is a central part of Northland PACE Senior Care Services. Participants, families and our Care Team determine the number of days you will attend the Day Center. Participants attend, see their doctor, participate in therapies and recreational activities, or just catch up with friends. A noon meal is served Monday through Friday. Day CenterThe Northland PACE Activities team present fun, creative and stimulating programs that get participants moving and thinking!
