With the assistance of the Family Alliance team individuals can design their day based on their own preferences. A typical day for one person may include a computer class, some morning tai chi exercises, work on a special art project, and a current events discussion while another person joins the music appreciation group, learns mind and body practices to relieve stress, including gentle yoga, plays some cards, joins the book club, or works in our award-winning garden and much, much more. The Adult Day Health Program is central to Family Alliance’s concept of active adult wellness. In their effort to encourage healthier lifestyles for all adults, Family Alliance provides comprehensive services to meet the needs of each individual. The Adult Day Health team includes registered nurses, certified nursing assistants and activity specialists, who will work with everyone to “reach” their personal potential.

This year we began serving seniors in our Adult Day program. This program provides a safe and comfortable environment for senior citizens who are unable to remain in their homes alone. Music, fitness activities, physical therapy, arts and crafts, and socialization activities are scheduled. Breakfast, a hot lunch and snacks are provided each day. Opportunities to spend time in the community are also scheduled each week.

The Adult Day Services program at the Condell Day Center was established for families who require all-day care and supervision for older adults living at home and needing assistance with the activities of daily living. Designed specifically for those who are ambulatory, functional in a group setting and under a physician's care, the program offers a safe, comfortable, friendly and supervised environment for health, wellness and recovery. In many cases, the Adult Day Services program is less costly than in-home care and therapies. Outpatient services, such as lab tests and X-ray services are reimbursed by insurance and Medicare.

Our Traditional Care adult day program is designed for persons who may have mild social, physical, emotional or cognitive needs, including ("but not limited to" )early to mid-stage dementia. A client in Traditional Care is relatively independent and needs little direction and/or assistance with activities of daily living. The Asher Alzheimer's and Dementia Care program is designed for persons with mid-stage Alzheimer's or other types of dementia, physical or cognitive impairments that require assistance with a variety of activities of daily living. The Asher client is typically in need of significant direction, cueing and personal assistance by staff. In addition to full activity staff, Traditional Care and Asher programs have both a full-time registered nurse to dispense medication, promote wellness and provide health education social worker to provide individualized care plans and emotional support. Kosher meals and snacks are served; special diets are accommodated.

Adult Day Service (ADS) provides on-site care for older adults, ages 60 or above, to promote their social, physical, and emotional well-being in a supervised and structured group setting. The program provides seniors with social, recreational, therapeutic, and culturally sensitive activities, including but not limited to arts and crafts, fitness classes, outings, and singing and dancing groups. Participating seniors receive transportation services to and from the center if living within the service area, nutritious meals, and regular health monitoring by a registered nurse. The program also offers respite services for family caregivers. This service is free for eligible low-income seniors. We also serve private pay participants.

Our Adult Day Care Center is a professionally operated program designed to assist older adults who don’t want or need the continuous care of a hospital or other long term facility, but may need help with self-care, require supervision, or would like to socialize with others. We offer a variety of health and social services in a protective, sociable and positive group setting. This program provides families with relief from caregiving responsibilities, emotional reassurance, assistance with their family member’s care, and short or long term attendance plans. Adult Day Care has an on-site Registered Nurse Director and Activity Coordinator (CNA), Monday through Friday, 7:00am – 3:00pm, except for holidays. Our nurse and CNAs provide assessments including blood pressure, weight, nutritional status, and note any changes in level of functioning. Our nursing staff also administers medication as ordered and may provide skilled nursing services and treatments with physicians orders, bathing and personal care as indicated or requested. We use a team approach to assure your loved one is well cared for.

Since 1981, Chicago Commons has helped seniors and disabled adults to continue to live in their homes and communities as long as possible, preventing or delaying the need for institutional care. As a leader in senior care in Illinois our purpose is to lengthen lives and build the well-being of seniors and adults with disabilities. Our Adult Day Service centers accommodates up to 130 adults daily and is open Monday through Friday. We operate with fully-trained staff and a Registered Nurse on duty. We provide nutritious meals, individual physical and mental health care, as well as daily physical, recreational and social activities. Transportation to and from seniors’ homes can be arranged in our wheelchair- accessible vehicles.
