Details: CentraCare is Calhoun, Kalamazoo and Eastern Van Buren County’s PACE program. PACE stands for "Program of All-inclusive Care for the Elderly." PACE is a long-term comprehensive health care program that enables frail elderly adults to live at home in their communities for as long as medically and socially possible. The Day Health Center provides many different recreational activities, hot meals, doctor appointments, and the therapy you need. CentraCare will provide transportation to and from the Center and for medical appointments set-up by CentraCare.
