Footprints in Time is a nonprofit, faith-based organization that serves as a multi-generational outreach to health compromised/socially isolated adults. We provide daytime care for caregiver's loved ones so they can take time for themselves. We provide opportunity for adults with unique needs to interact socially with peers by sharing in stimulating activities that support physical, emotional, mental and spiritual well being. We enrich lives, show individuals that they are important members of the community and offer encouragement and practical support. Our program is designed for adults who are isolated and lonely or who can no longer live on their own. It includes acommodation, board coordinated care services and a full day of life enrichment activities to help participants continue to live at home with their loved ones. We strive to provide a safe, supervised haven for any part of the day or night (Adult Day Program) and short-term stays (Respite Program) for family caregivers' respite, post-hospitalization rehab or hospice. Angel Gardens Adult Day and Respite Program offers a win-win solution for everyone in the family. Participants enjoy opportunities for socialization and engagement, which promote their well-being and independence, while receiving apprpriate functional assistance in a supervised, nurturing environment. Caregivers benefit by getting a much needed break from caregiving duties, knowing that their loved one is in good hands.

Senex is a social and recreational day program dedicated to serving older adults with functional needs in out community by becoming involved in the activities offered by the City of Farmington Hills Adults 50 & Better. The Senex program provides opportunities for individuals to maintain or improve their functional abilities in a safe, supervised structured environment. This reduces isolation, enhances self esteem and maximizes their potential. Our participants include older adults who are experiencing a decrease in physical, mental of social functioning. Appropriate diagnoses may include isolation, Alzheimer's disease, Parkinson's disease and stroke survivors as well as other challenges.

Adult Day Service programs are community-based group programs that support families by providing a variety of social and health services to the person needing assistance. One reason to use an Adult Day Center is to give yourself a break from caregiving. Some caregivers worry that their loved ones will resent participating in such a program, while others feel guilty at the thought of leaving their loved ones in another person's care. Care Resources is a Program of All-inclusive Care for the Elderly (PACE®). Care Resources was developed as a health care program for older adults needing long-term care and wishing to age in place. As we age, medical problems become more complex and last longer. Care Resources is responsible for all primary and specialty care for those who are enrolled. PACE Eligibility: 55 years old or older; Resident of Kent County or the following zip codes in Ottawa County: 49418, 49428, 49435, 49534, 49544; Able to live safely in the community when you join PACE. PACE is different from any long-term care option. We partner with you to stay independent and in good health, as you age. We have health care professionals who watch for changes in your condition, provide treatment, and encourage you to stay independent. When you enroll in the program, you will receive services unique to your own needs. The PACE model is an all-inclusive health care program that promotes better care and better health.

CentraCare is Calhoun, Kalamazoo and Eastern Van Buren County’s PACE program. PACE stands for "Program of All-inclusive Care for the Elderly." PACE is a long-term comprehensive health care program that enables frail elderly adults to live at home in their communities for as long as medically and socially possible. The Day Health Center provides many different recreational activities, hot meals, doctor appointments, and the therapy you need. CentraCare will provide transportation to and from the Center and for medical appointments set-up by CentraCare.

Family Ties is an Adult Day Center located in Iron Mountain in Dickinson County, Michigan and is open to individuals diagnosed with Alzheimer's and Dementia's, as well as other debilitating diseases. Family Ties is a structured program providing stimulating activities and socialization as a vital part of the center's daily routine. The setting allows the individual the opportunity to gain a feeling of accomplishment from participating in the activities of the center. Family Ties Adult Center also provides respite for the caregiver. Respite allows the caregiver time to accomplish daily responsibilities while their loved one is cared for in a safe, secure, and stimulating environment. The center creates an option to families other than placement in a long term care facility. This option allows the participant the opportunity to remain in their own home, living in familiar surroundings, while at the same time supporting the family's needs. The care giver has time to take care of the daily necessities of living while getting some much needed respite from the care of their family member.
