Details: A Grace Place Adult Care Center cares about the health and well being of your loved one as much as you do. We can do this together. We have been offering a day-based alternative to full-time nursing home or institutional care since 1967. A Grace Place operates with a person-centered philosophy. We will do whatever it takes to meet individual needs and interests. And as the client’s needs change or increase, we adjust or increase our support. We do not discharge the client. We provide daytime services for adults with disabilities and age-related conditions, and respite and support for their caregivers. Our clients and their families reflect the diversity of our community. A Grace Place has an open admission policy, which means we welcome everyone, regardless of diagnosis or funding ability. We offer two models of care, Adult Day and Connections. Each features specialized day programs for adults with a range of abilities and disabilities.
