Details: The Schulman and Schachne Institute’s Adult Day Health Care Program offers health care services and activities to the community for persons who have varying health care needs, enhancing their health status and allowing them to remain in the community. As a community-based long term care program, we provide comprehensive health care to the frail elderly, chronically ill and disabled individuals in a congregate day setting. The Schulman and Schachne Institute for Nursing and Rehabilitation Adult Day Health Care Program is designed to provide a safe and supportive environment. By offering health care services, activities, education and social stimulation, we enable adults to remain in the community, living at home as long as possible, despite physical or cognitive impairments.
