AmeriCare Adult Day Health Care is a licensed community-based program providing a variety of health, therapeutic, and social services to medically, socially or psychologically eligible adults. Currently, over 300 centers exist in many areas of the state of California. ADHC services became an optional Medi-Cal benefit in 1978. Since that time, ADHC centers have successfully provided services to the community. AmeriCare Adult Day Health Care Center has been licensed by the California Department of Health Services and certified by the Department of Aging since October of 2002 to provide services to 135 participants daily. AmeriCare Adult Day Health Care Center is a certified Medi-Cal provider. Title 22, Division 3, Chapter 5 outlines the Adult Day Health Care Medi-Cal certifications and licensing regulations. AmeriCare Adult Day Healthcare Center provides a safe and nurturing environment for adult individuals of all ages with medical and psychological challenges. Many of our participants are frail and elderly. AmeriCare’s program assists people who wish to achieve their optimal level of health and independence while enjoying daily activities. Adult day health services supplement the care provided at home. It enables a caregiver to work and provide support for one’s family while simultaneously caring for one’s loved one at home for as long as medically, socially and economically possible. AmeriCare also provides structure, support, and safety to the lives of those living independently or in a Licensed Residential Care facility. AmeriCare’s multi-disciplinary team (MDT) is comprised of a nursing dept., medical director, social services dept., physical therapist, occupational therapist, and activity director. Based on the participant’s needs, other professionals as psychologist or/and registered dietician or/and speech therapist, may be included to the MDT team to decide and implement Individual Plan of Care. The nursing department includes several RN’s, LVN’s, and CNA’s. They oversee medication management and educate participants on their medication and its side effects. They also communicate with our mental health participants’ primary doctors as well as their psychiatrists regarding medication, symptom exacerbation, and anything else concerning health that requires reporting. The importance of helping adult individuals of all ages with medical and psychological challenges to remain in their own homes cannot be over emphasized, Not only does Adult Day Health Care provide needed services to assist and keep persons from having to be in a higher and more expansive level of care; research tells us that persons who remain with their family or familiar living setting not only have a longer life, but a higher quality of life. |
