Here at Presentation, we serve a diverse group of participants including Chinese, American, Korean, Filipino, and African American. Our staff is as diverse as our community members and Cantonese is the number one language spoken at the Center. Thanks in large part to our ongoing relationship with St. Anthony Foundation, members of the Presentation Day Health community enjoy daily intergenerational activities with visiting high school volunteers. |

At Golden Gate Day Health, we serve participants who represent the rich cultural heritage found in San Francisco. Community members are Chinese, Latino, Filipino, Vietnamese, Cambodian, Asian Pacific Islander, Japanese, African American, and American. Because we serve such a diverse population we have a multilingual staff, which allows us to easily communicate with all of our participants. |

Often times, caregivers or family members of elders feel guilty about leaving their loved one alone for the day. Self-Help for the Elderly offers an adult day care service that takes care of your loved one, so that you don’t have to worry anymore. This elder care service serves frail seniors and adults with physical impairments, mental impairments or dementia that cannot be alone for the duration of a day. They are provided with care and companionship as well as assistance and supervision during the day. The adult day care program allows family members or caregivers a few hours of relief during the day to go to work or fulfill other obligations. They can trust that their loved ones will be cared for and safe throughout the day. This program is meant to prevent seniors from being put in living facilities by providing alternative care. We strive to promote the social, emotional and physical well-being of our seniors, while helping them maintain or restore their essential functions. The adult day care facility is located at the Jackie Chan Center in San Francisco. |

Since 1999, our L'Chaim Adult Day Health Center has provided outstanding medical, rehabilitative, and social services to thousands of elderly and disabled clients. At L'Chaim, we offer a variety of services under one roof, specially tailored to meet individuals' and families' needs in a warm, supportive, and friendly atmosphere. Our staff of professionals provide nursing and personal care; case management; physical, occupational, massage, and speech therapy; nutritional counseling; and recreational and socialization activities. Transportation, hot lunch, and snacks are also provided. Our goals are to maintain and enhance our clients' health; help them to remain independent in their homes and community for as long as possible; and reduce the likelihood of their falling, visiting the emergency room, or remaining hospitalized for prolonged stays, and avoiding premature institutional care. Center hours are 9:00 am - 1:00 pm and 2:30 - 6:30 pm, Monday through Friday. Staff are available 8:30 am - 7:00 pm on those days. We offer a sliding scale fee and free services to Medi-Cal participants. L’Chaim, located at 2534 Judah Street, is in the heart of San Francisco’s Sunset District. Our staff is bilingual in Russian and English. |
