Redwood Coast PACE is an all-inclusive health plan for older adults age 55+. We provide medical, social, nutritional, rehabilitation, and support services for frail adults who: Are age 55 and older; Need a nursing home level of care but who wish to remain at home or in the community. (The state of California determines/certifies each applicant’s need for a nursing home level of care.); Can safely live in the community with no harm to themselves or others, and Reside in the Redwood Coast PACE service area - Eureka, McKinleyville, Arcata, Bayside, Blue Lake, Carlotta, Ferndale, Fields Landing, Fortuna, Hydesville, Korbel, Loleta, Rio Dell, Samoa, Scotia, or Trinidad. Redwood Coast PACE is modeled after a nationally recognized health model called the Program of All-inclusive Care for the Elderly (PACE). Redwood Coast PACE is licensed through the California Department of Health Care Services and the Centers for Medicare and Medicaid Services (CMS). |

Adult Day Health Services (ADHS) has been authorized by the state to provide Community Based Adult Services (CBAS) and continues to receive State Medi-Cal funding. We are still providing the same great care that you have come to expect and we’re ready to enroll new participants. ADHS is a medically licensed, therapeutic day program that provides a caring alternative to nursing home care for those who do not need 24-hour skilled nursing. The program is designed to help people stay mentally and physically active, reduce isolation, improve their health, and prevent decline of their abilities. People typically attend the program one to five times a week. Services include therapeutic activities, games, health monitoring, social services, meals, dietary counseling, physical, occupational and speech therapies. |

Sequoia Center is a day program offered by HCAR. We provide training and support in the areas of independent living, vocatioal skills, recreation and leisure skills, as well as community integration. We also offer a tutor service. The people here enjoy healthy Nutrition class-training, we are involved in volunteer work in the community, and we have very good Shredding operation at our site where people can work and earn a pay check, if they choose. |

Crestwood Behavioral Health Inc. has been providing residential mental health programs at its Eureka campus since 1979. The Eureka campus is home to a mental health rehabilitation center, an adult residential facility, a day rehabilitation program, a mental health recovery clinic, and a residential care facility for the elderly. The focus of our programs centers on recognizing the unique gifts, interests and challenges of each person we serve, and supporting creativity, wellness and empowerment throughout the recovery journey. The campus location, in the heart of California Redwood country, allows clients to tap into the mental health benefits that the surrounding natural environment offers. |

The Carole Sund Center opened in 2000 and provides day program services to adults with developmental disabilities. The program was relocated in 2013. It is located on over 4 acres of agricultural land. Our participants are "farming for their future". In this acricultural based environment the participants can earn money working the farm by selling the fresh vegetables and eggs to community members and their residential facilities. Along with working the farm, landscaping and community recycling, many of our individuals are enjoying employment opportunities working with the Tri-City Weekly. The participants are not only helping to meet the needs of the local community while developing and maintaining a wide variety of skills, they feel a sense of pride and accomplishment from their work and really appreciate a paycheck! |
