Details (Max 1000 characters):: Adult day health care is a community based long term care program that provides comprehensive health care services in a congregate day setting. Registrant's needs are assessed and met through an individualized plan of care that is developed and implemented by an interdisciplinary team of medical professionals, including the registrant's personal community physician. Adult day health care program registrants have all of their health care needs assessed, met, or arranged for at the center. The program of comprehensive, integrated care leads to stabilization of chronic health conditions and reduced incidence of costly physician and emergency room visits. Registrants are provided the medical and social services they need to remain at home and a part of the community. The program is designed to meet the needs of chronically ill, frail elderly and disabled adults who require certain primary, preventative, diagnostic, therapeutic, rehabilitative or palliative services. A person might consider adult day health care: If existing and necessary care giver support is eliminated or compromised; When a medical condition develops or a new diagnosis is rendered which requires acute or chronic medical care; Following an accident or acute episode; If an existing condition is exacerbated; When hospice care is required; If restorative therapy is needed following an acute episode; During end stage disease; An individual must be Medicaid eligible, willing to pay privately, or have a private insurance source that will pay for service.
