We provide medical, social, nutritional, rehabilitation, and support services for frail adults who: Are age 55 and older; Need a nursing home level of care but who wish to remain at home or in the community. (The state of California determines/certifies each applicantâ€™s need for a nursing home level of care.); Can safely live in the community with no harm to themselves or others, and Reside in the Redwood Coast PACE service area - Eureka, McKinleyville, Arcata, Bayside, Blue Lake, Carlotta, Ferndale, Fields Landing, Fortuna, Hydesville, Korbel, Loleta, Rio Dell, Samoa, Scotia, or Trinidad. Redwood Coast PACE is modeled after a nationally recognized health model called the Program of All-inclusive Care for the Elderly (PACE). Redwood Coast PACE is licensed through the California Department of Health Care Services and the Centers for Medicare and Medicaid Services (CMS).
