Details (Max 1000 characters):: Sheepshead Adult Day Health Care Program is in a separate location across the street from our facility, at 3900 Shore Parkway, Brooklyn, NY. Our program provides respite for caregivers so they can go shopping, go to work or relax while their loved one receives warm and loving care in a safe, friendly environment. Our center operates during normal business hours five days a week. We are a medical model program. Our staff includes a social worker, registered nurse, certified nursing assistants and recreation activity personnel who follow a care plan that has been customized for each participant. The care plan is developed in conjunction with the personal physician who is kept abreast of the patient's health status. Participants have been shown to improve cognitively and physically when they attend an adult day health care program, which fosters independence, provides social interraction and greater structure to the participants' daily schedule.
