Details (Max 1000 characters):: Our Adult Day Health program monitors each individualâ€™s medical condition while offering a wide range of social, leisure and community activities. The program provides participants with an inter-disciplinary team approach to care, and also includes daily transportation to and from the center and a balanced and nutritional lunch. An on-site sensory room provides stimulation or relaxation for those who need it. Upon an adultâ€™s entry into the program, the director of nursing, social worker and activity coordinator complete a plan of care which is reviewed and updated quarterly. The program staff and each participant also develop a goal together that is designed to foster a greater level of independence. A consultant dietitian, physical therapist and pharmacist also assist in the plan of care and provide direction to program staff persons, participants and their caregivers. The New Jersey Department of Health monitors this Medicaid-funded program. Medicaid care managers authorize each adultâ€™s participation. In order to be eligible, an adult must have a medical need that requires daily medical monitoring or receive assistance with three activities for daily living.
