Details (Max 1000 characters):: Our objective is to provide social day health programs with a caring staff which enhances the quality of life for our clients and caregivers. We strive to offer social outlet that provide continued enrichment and opportunity for our elders to make new friends. The program will provide six hours of nursing care and health supervision using an individualized plan of care. Two meal, and one nourishing snack will be prepared in accordance with the physiciansâ€™ order, and served daily. A therapeutic program inclusive of Physical Therapy, Occupational Therapy, Speech Therapy, as well as educational, social, and recreational services, will be offered as needed, to obtain and maintain the participantâ€™s maximum level of functioning. Group and family counseling will be provided with referrals made to community agencies if appropriate.
