LIFE helps participants experience peace of mind with the assurance that all their medical cares will be met. Nationally recognized as PACE (Program of All-inclusive Care for the Elderly), LIFE includes elements of the traditional healthcare system, but with LIFE it's not just a system, it's a lifestyle. Dedicated to enabling participants to enjoy life at home, LIFE uses an interdisciplinary medical team approach to care. This team is comprised of physicians, nurses, therapists and other health care professionals who work closely with each participant. To maintain or improve personal and physical health, the team works with the participant and caregiver, assessing each participant's needs and developing a care plan based on the participant's goals. LIFE Lutheran Services, Inc. has contracts with many specialists and other providers in the community to ensure that participants get the care they need. Regular evaluations are conducted to closely monitor participant's achievements and progress. Continually adjusting the care plan for optimal care, the all-inclusive LIFE model has shown noted improvement of participant health and happiness.
