Our adult day health centers are a preferred alternative to nursing homes, assisted living facilities or institutional care, providing engaging therapeutic activities, opportunities for socialization and expert health and nursing care dedicated to helping our clients improve daily functioning or delaying the onset of chronic diseases. Low country Active Day center is located in Goose Creek, South Carolina. The center services clients in the tri county area ( Charleston, Dorchester, Berkeley). The center cares for clients with varying mental challenges beginning at the age of 18 years of age. The center offers structured activities to meet each clients' needs. We offer a morning snack, an afternoon snack and a full coarse lunch. All menus are planned a Registered Dietitian. The center is staffed daily with a Registered Nurse, a Licensed Practical Nurse , and 4 personal care aides. The center is able to offer tube feedings, medication administration, monitoring of blood sugars and blood pressure. The center also provides hands on care to allow their clients to complete the activities of daily living. Within the center there is a Professional Rehab team; making physical therapy, occupational therapy, and speech therapy only steps away.
