Details: Adult Day Health Care Services located at Sullivan County Adult Care Center. Adult Day Health Care Services is designed to meet the medical, physical, mental, emotional and social needs of adults in a congregate setting. The program goal is for the registrant remain in the community with the assistance of the family and/or support services as long as their needs can be met and safety maintained. The registrant is linked with needed services. On-site services include nursing, medical social worker, rehabilitation therapies (physical therapy, occupational therapy, and speech therapy), podiatry, nutrition, and a certified nursing assistant.
