Details: The St. Barnabas Adult Day Health Care Program is designed to meet the needs of individuals who are physically, emotionally or mentally impaired. We offer medical, nursing, social work, restorative and rehabilitative services. Our seven-day -a-week program offers diversified therapeutic activities that promote stimulation, socialization and enjoyment in a safe and structured environment. Our Medical Model Program provides a case management approach, resulting in an individualized plan of care tailored to meet each participant’s specific needs. Applicants for the program may be referred by their private physician or other community resources. Every applicant will have a full admission assessment which will include a medical needs determination that the applicant is expected to need continued service for a period of 60 or more days. Program participants are screened for physical therapy, speech, dental and ophthalmology services. Additional medical services are available on site, with links to the individual’s private physician or hospital-affiliated specialty services. Nurses are attuned to the medical conditions of participants and ensure maintenance of current medications, clinic and physician appointments, yearly immunizations and TB surveillance.
