Details: Bay Park’s Adult Day Care Program is an alternative to nursing home placement where the frail elderly and/or disability young adults who are being cared for at home to spend time away from home in a supportive environment. Our program also gives participants the opportunity to meet and establish supportive relationships with others who experiencing similar life situations, medical conditions & psycho-social needs. We provide services 7 days a week to maintain participant’s well-being in the community. We are committed to providing a nurturing and active adult environment as well as medical monitoring, education, support and guidance. Eligibility for our Adult Day Care Program requires that the applicant has medical condition that needs monitoring or safe secure program due to cognitive losses, including Alzhheimer's disease.
