Details: Avalon Gardens Rehabilitation &Health Care Center is a unique multi-generational campus of care located on 22 beautiful acres in Smithtown, Long Island. Our Adult Day Health Care Program is available for eligible frail elderly and chronically ill or disabled adults who require long term care services, but are able to remain in the community. Registrants are assessed by our interdisciplinary team of medical professionals who develop a customized plan of care. Our goal is to help stabilize chronic health problems while preventing additional health issues so registrants can remain at home in the community as active and healthy as possible. Our professional and experienced team consists of registered nurses, social workers, licensed physical, occupational and speech therapists, registered dietitians, clinical psychologists and recreational staff. All health care is coordinated through the individual's personal physician in the community. The beautiful Adult Day Health CareCenter at Avalon Gardens offers registrants comprehensive nursing, medical, diagnostic, social, nutritional, rehabilitative and personal care services.With door-to-door transportation, hot lunches and nutritious snacks, accessible health care and healthy dining are just minutes away. Registrants enjoy the camaraderie of socialization, participating in a variety of activities with the benefit of monitored and supervised health care. Participants enjoy an extensive arts andcrafts program in our art studio and cooking classes and food preparation in our beautiful kitchen. Music and dance therapy and exercise are popular activities. In addition, our computer center located in the Adult Day Health Care Recreation Center enables participants to surf the Internet and email family and friends.
