Knowing how to provide the proper care can be even harder. We offer respite care and adult day services at select Trilogy health campuses to give daily caregivers a break and provide some tips for coping. These services are designed to provide a safe and secure environment for short-term stays of a few hours or an entire day. Additionally, some campuses offer adult night services, for caregivers who need assistance or a break during the evening hours. Our Adult Day/Night Services offer a structured program of activities, health monitoring and socialization for adults who have physical or cognitive challenges and/or who may require supervision or assistance with personal care or other daily living activities. Adult day services can also benefit family caregivers by enabling them to maintain their work schedule or to receive needed respite, and by providing them with direct services like support groups and educational programs.

Our adults receive personal care and assistance, medical consultations by an agency nurse and social/recreational activities. Participants engage in socialization, exercise, arts & crafts, current events discussions ad decision-making training. Transportation is available based on the pick-up location of the individual. Renewed Horizons is a developmental program for individuals who are 50 years of age or older, are a Porter County resident (or a resident in a surrounding county), have been diagnosed with Alzheimerâ€™s or other physical/mental disorders, are able to be medically managed, are free of communicable diseases and are able to provide appropriate funding. Connections is a component of curriculum in Renewed Horizons and its purpose is to help participants maintain as much cognitive and physical functioning as possible. This set of over 40 activities fosters cognition, socialization, communication and manual dexterity. Connections is scheduled twice a week, and since several are offered at a time, participant choice is once again a factor.
