At VNA Homecare, we know that being able to socialize with peers and enjoy activities in a safe environment is important for those with long term care needs. We also understand that caregivers need a respite from the task of caring for their loved one. That’s why we’re pleased to offer adult day services that can help both participants and caregivers. VNA Homecare’s Adult Social Day Program offers people who have long-term care needs with a safe, protective home-like setting outside of their homes during the day. Consistent, therapeutic programs and approaches, designed with the functionally impaired in mind, provide a variety of health, social and supportive services. Activities are structured and include discussion of current events, arts and crafts, exercise, games, meals and snacks. Once enrolled in the VNA Homecare Adult Social Day Program, everyone can rest assured knowing that care is being delivered in a safe, socially stimulating environment. |

A number of our participant's caregivers' work full time jobs outside the home. We have listened to stories from caregivers feeling guilty leaving loved ones home for extended periods of time unattended, in order to keep their employment. We have empathized with caregivers who are struggling to fulfill promises of caring for their loved ones at home and preventing admissions into a nursing home setting. As your loved one's illness progresses it becomes more difficult for you to give the care needed. Participants and their family members report feeling rejuvenated, as well as a sense of calm in the household at the end of a busy day. The participants say they no longer feel as if they are a burden to their family members. The caregivers report a sense of peace of mind knowing their loved ones are cared for during the day. Central Park provides enjoyable, supervised recreational activities such as, entertainment, music, daily exercise, cognitive stimulation activities, and creative art programs. Allowing for a chance to socialize with other adults. Nursing services are available with a Registered Professional Nurse, who along with a Licensed Practical Nurse and Certified Nursing Assistants provide medication administration, wound care and personal care, such as bathing, toileting assistance and incontinence care. Central Park offers for the participant a safe, secure environment to spend the day with enriching activities and socialization. For the caregiver it reduces the feeling of guilt, while being able to retain a job outside, or time to be able to take time away from the 24-hour responsibility, which comes from being the primary caregiver. |

Since 1987, the Kirkpatrick Day Program has provided high quality care in a safe and secure environment located near Downtown Syracuse. Day services like ours provide reliable support and care to its participants, while enhancing the overall quality of life for caregivers. Our staff of trained dementia care professionals designs activities based on the individual needs and interests of the participant, with the goal of stimulating cognition and maintaining memory. Simply put: Caring Is Our Passion. Most adult day services are open to any older adult that wishes to attend. Admission to the Kirkpatrick Day Program is reserved for individuals with Alzheimer’s disease or another form of dementia, including Pick’s disease, Lewy Body dementia, Parkinson’s disease, stroke-related or vascular dementia, and Huntington’s disease. A doctor’s diagnosis is not required for attending, however we encourage anyone who suspects that dementia affects them or their loved one to consult a medical professional. |

Established in 1971, the St. Camillus Adult Day Health Program was one of the first medical day programs in New York State. In keeping with that same pioneer spirit, our program has continued to expand and to develop an array of services designed to enable adults with medical problems to live in their home as independently and for as long as possible. This is accomplished by developing a comprehensive individualized plan that coordinates our medical day program services with any additional community services needed to meet this goal. In addition to the provision of vital services for the individual, the program can offer education, support and a respite for those caring for their loved ones at home. The Adult Day Health program allows family members the chance to take care of themselves, knowing their loved one is being well-cared for in a safe environment. In April 2013, the new Social Adult Day Health Program opened. The mission of the program is to assist adults with neurological impairments, such as stroke, traumatic brain injury (TBI), and chronic illness, such as Parkinson’s disease in maintaining their optimum health and independence in the community. The goal of this program is to enable persons with neurological conditions to obtain support and develop strategies to enhance their ability to remain living at home. |

Loretto’s Daybreak Adult Medical Day Program serves the medical needs of Central New York adults over the age of 21 in a comfortable and safe day setting up to six days a week. Daybreak is designed to help adults maintain their highest level of independence while continuing to live in their community. Program participants must have a medical need in order to qualify for the services provided by Daybreak, which are covered by Medicaid and attend Daybreak for a minimum of five hours per day, six days per week. |

The Salvation Army’s Senior Center and Social Day Program provide opportunities for socialization and wellness that support independence for our community’s seniors. The Senior Center provides a warm and welcoming atmosphere that fosters a sense of community. Nutritionally balanced breakfasts and hot lunches are available for all participants. Seniors have the choice to participate in many programs, including computer instruction, yoga, exercise and strength training, falls prevention, diabetic support, health monitoring and education, ceramics and crafts, music, spiritual programs and personal enrichment. Support is provided for full participation by frail elderly who are at risk for institutionalization and/or nursing home care. Available services include transportation, referral to case management services and other community resources, assistance on outings and trips and caregiver support/assistance. A full time Nurse LPN is on the Senior Center staff. |

The Medical Day Health Program is offered Monday through Friday from 8:00 AM - 4:00 PM and serves adults 16 years or older who: Are returning home from a hospitalization and/or short/long term rehabilitation; Require additional medical, rehabilitation, and support services to maximize health status; Can increase independence so they can remain safely in their home; Have a progressive or chronic illness that has resulted in a gradual decline in health; Require ongoing medical services and coordination of multiple services to maximize health and the ability to remain at home; Require medical day services as a component of a waiver program (TBI or NHTD) or to supplement home health services. |
