We have a full team of providers dedicated to maintaining elderly patients in as independently capacity is possible. This includes, if necessary, visiting nurses, physical therapists, dietitians, occupational therapists, psychologists, and if the patient is completely homebound, a visiting nurse practitioner. We also operate an adult day health facility to provide respite care for families of eligible patients on a daily basis.

"Senior Day Services" is a Social Model Adult Day Care located in the lower level of the Ava Dorfman Senior Citizen's Civic Center. The Social Model Adult Day Care is provided to allow individuals to remain independent in the community and active at the Senior Day Care Program with a goal of promoting their well-being. The Social Model is ideal for individuals who require some assistance or supervision with self-care, such as frail, isolated seniors, stroke victims, individuals with Alzheimer's disease and other age-related problems.
