Heart of the Village (HOV) is a new program in NW Rochester, MN whose focus is to offer services to individuals with brain injuries, neurological disorders as well as those with mental health disorders. HOV hopes to provide a professional yet home like environment for participants to feel comfortable and accepted as they work to regain some if not all of the sense of self they had prior to their brain injury. Our program offers support and guidance to participants that have an interest in improving on skills needed to be more independent in areas of personal growth, relationship building, cognitive abilities, community involvement and independent living. Through group activities designed to challenge each individual at their own speed and interest level HOV staff will guide growth with budgeting, use of community resources, prevocational skills, self-awareness, health, and life skills development. Participants will be expected to have clear goals as they enter into the program and will be encouraged to participate in activities that will help them to achieve these goals, though involvement in daily groups and presentations by staff and community speakers and resources will always be optional to each participant. As HOV is designed to promote independence to the highest level, this program may not fit the needs of all individuals with brain injuries. The program does ask that each participant be independent in basic mobility, have a managed incontinence program, be free of communicable diseases, and require no more than one person assist. Participants will also be expected to show skills needed to comply with company, program and state licensing policies. |

The Next Step Adult Day Program and Wellness Center is available to adults (18) eighteen years of age and older who have been diagnosed with a TBI, ABI, neurological disorder or mental illness. All applicants are considered regardless of race, color, sex, national origin or sexual orientation. An initial interview and assessment will help determine the potential participant's appropriateness for the program. The following criteria must be present to be considered for the program: Is physically, mentally and medically safe to participate in programming; Does not have a communicable disease; Does not cause harm to self or others; Cooperates with staff in all areas of programming and documentation; Has no unmanageable wandering; Is able to be redirected; Manageable incontinence; Is mobile with or without a device; Can transfer by his/her self; Has set goals and is motivated to achieve those goals; Has a motivation to participate in all areas of programming. The Next Step Adult Day Program and Wellness Center is for individuals who can benefit from: community integration, personal growth, socialization, activities, medication administration, exercise and supervision. |

Abilities Unlimited – or AU – is a licensed structured day program for adults who have acquired physical and/or cognitive impairments as a result of traumatic brain injury, stroke, multiple sclerosis, accident, or a variety of other conditions. The program offers a variety of daily activities tailored to meet their specific needs, interests and schedules. All activities are designed to enhance the participants’ cognitive, social and communication skills. Program components include cognitive and physical fitness, personal care and wellness, community integration, leisure education, peer support, behavior management, communication, and social skills training. AU participants have many opportunities to volunteer. Currently, we volunteer at the Ronald McDonald House and at Channel One Food Shelf. We provide a blend of recreation and leisure activities, with community integration as a focus of our programming. |
