Providence ElderPlace is an innovative care model known as PACE (Program of All Inclusive Care for the Elderly), a federally recognized, comprehensive care program that offers health, housing, social service and care coordination solutions to older adults. Providence ElderPlace in Laurelhurst is one of eight Health and Social Centers where members receive integrated PACE services. The ElderPlace in Laurelhurst site is located in the Laurelhurst neighborhood of Portland, kitty-corner from Providence Portland Medical Center. ElderPlace participants receive medical and social services while attending the Laurelhurst Health and Social Center. All authorized services are covered in full for providence ElderPlace participants. PACE participants must receive all needed health care, including primary care and specialist physician services (other than emergency services) from the PACE organization or from an entity authorized by the PACE organization. PACE participants may be fully and personally liable for the costs of unauthorized or out-of-PACE services. Emergency services are covered. |

Respite and Adult Day Care is a short-term stay in a fully-furnished apartment, from just a few hours or up to 90 days. Our programs allow the caregiver time away from the heavy demands placed on them when caring for another and is a great way to experience our Regency Park lifestyle. The primary reasons for a respite stay are to ease the transition from hospital to home, or to provide an alternative living situation in the event a primary caregiver is temporarily unavailable. Respite and Adult Day Care residents receive the same services, care and personal treatment that our long-term residents receive. |

Emerson House understands that caring for a loved one at home can sometimes be both mentally and physically challenging. Our Adult Day Program, and others like it, has been shown to lessen the prevalence of caregiver exhaustion while helping participants remain in the home for an extended period of time. The respite provided by Adult Day Programs allows caregivers the time to care for themselves, and in turn furthers their ability to care for their loved ones. For working caregivers, knowing that your loved one is in a safe, stimulating environment can provide the assurance you need to maintain your obligations and responsibilities outside of the home. Our Adult Day Program is designed to provide a welcoming environment for older adults. Our stimulating and interactive programming was designed with our participant’s interests in mind. Daily structure is planned by evaluating our participant’s social history and recreational interests, allowing us to provide a varied activity calendar while engaging all of our participants. Activities include time for physical movement and exercise, musical involvement, arts and crafts, recreation and games, social stimulation, monthly outings, and celebrations. Nourishing meals and snacks are provided to sustain our participants through their full and engaging days. Our specially trained staff at the Emerson House Adult Day Program has been selected for their commitment to ensuring purposeful experiences for our participants. One way we achieve this goal is by allowing participants to determine their level of involvement. Daily activities are designed with the hopes of including all participants, but we recognize the need for flexibility. Participants in the program can choose to engage in activities, sit quietly with a book in the reading nook, or engage in a lively conversation in one of our comfortable seating areas. Our Day Room is a warm and friendly space for participants and their families. The attached outdoor courtyard is open for gardening or some quite time alone weather permitting. The bank of picture windows along SE Division Street provides a source of constant visual stimulation and creates a connection between participants and their surroundings. |
