With two locations, Sentara PACE – in Portsmouth and Norfolk – recognizes that older people often have medical problems that last for long periods of time and require services from several different sources. Of great concern is the fact that, as many people age, they become increasingly isolated from the support, companionship and resources of the community at large. Sentara PACE is that important link; assuring participants receive the care they need, when they need it. This program is a comprehensive health care and supportive services program for frail seniors who wish to remain in their homes and community. It is an alternative to nursing facility eligible individuals, but who would prefer to remain at home with services coordinated by Sentara PACE. The program is one that provides total care for participants, including comprehensive medical and rehabilitative services, in-home services and transportation. Sentara PACE is offered in Norfolk, Virginia Beach, Chesapeake, Suffolk and Portsmouth. The program is modeled after a national replication project called PACE, or Program of All-Inclusive Care for the Elderly and is an approved Medicare and Medicaid provider. It is operated by Sentara PACE and is a program of the Commonwealth of Virginia. All services must be furnished by or authorized by the PACE organization and participants may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services.

The Adult Day Services (ADS) is a state licensed, Veteran’s Administration approved daycare program for adults over the age of 45 with chronic conditions requiring daily supervision, and our licensed caregiver respite program operates each weekday. This community based service helps maintain the family unit and decreases the need for more costly institutional services. Client diagnosis includes Alzheimer's, dementia and related disorders, cardiovascular disease, severe depression and many others. This service helps keep families together and prevents premature placement in nursing homes. Studies prove that mental and physical stimulation drastically improves quality of life for people living with Alzheimer’s and dementia. Our Adult Day Service offers mind stimulating and physical activities for our participants’ wellbeing.
