LIFE (Living Independence For the Elderly) Lutheran Services is a creative program encouraging healthy living at home, designed to offer opportunities for seniors to reside safely in their own communities, surrounded by familiarity. This team is comprised of physicians, nurses, therapists and other health care professionals who work closely with each participant. To maintain or improve personal and physical health, the team works with the participant and caregiver, assessing each participant's needs and developing a care plan based on the participant's goals. LIFE Lutheran Services, Inc. has contracts with many specialists and other providers in the community to ensure that participants get the care they need. Regular evaluations are conducted to closely monitor participant's achievements and progress. Continually adjusting the care plan for optimal care, the all-inclusive LIFE model has shown noted improvement of participant health and happiness.
