(REAL Inc.) has a contract with the Department of Human Services and is licensed by the Department of Health to provide daily activity health services to SSI or income eligible participants. To qualify persons must have functional limitations that require some type of therapy through a doctor’s order. At our Adult Day Care Centers we provide recreation, transportation, well balanced meals by a dietician, snacks, and an LVN on site, etc. |
