Details: Fort Tryon Center’s Social Model Daycare provides a structured environment where people with dementia or declining in cognitive ability can socialize and interact with one another in a secure, supervised setting, while all their day to day needs are being cared for by a well-trained and dependable staff. Door- to- door transportation is available and 3 nutritious meals and snacks are served daily. Fort Tryon Center’s Medical Model Daycare program is designed for persons with medical problems, including dementia. The Medical Model Day Care program provides many benefits of a skilled nursing facility, while allowing participants to live at home. In addition to medical assistance, participants receive help with all activities of daily living, including eating, bathing, grooming, toileting and ambulation.
