Details: Our Adult Day Care Center, located in Queens, NY, is a medical model program - a community within a community of peers, and friends--an alternate family. We are committed to providing a nurturing and active adult environment as well as medical monitoring, education, support and guidance in the care of your family member. Eligibility for our adult day care program is a medical condition that requires monitoring, physical therapy or a safe secure program due to cognitive losses, including Alzheimer's disease. There is always a team of professional staff on hand to meet the needs of the participants. Daily, our staff consists of a registered nurse, certified nurse's assistant, a social worker, physical and occupational therapists, dietitian, and recreation staff. Podiatry care is provided. Dental, ophthalmology evaluations and speech therapy can be arranged, as well as transportation to these appointments. Case management is coordinated and provided through our comprehensive care planning process. The Medical Director monitors all programs and services. Hillside Manor's Adult Day Care Program, may be the perfect alternative to in-patient stay or continuum of care after a rehab stay. Involvement in Adult Day Care may assist in the delaying of placement providing a safe, nurturing environment while loved ones work.
