Details: Our goal is to improve the quality of life for the seniors in our community and provide family support. We offer a variety of medical, rehabilitative, social and recreational programs. Our experienced staff will develop personalized care plan to meet the individual needs of each participant. Hot Meals and snacks are prepared daily in accordance with Kosher Dietary Laws and individual requests. We are open Sunday through Friday. However, days of participation will vary depending on individual needs and circumstances. We arrange for door to door transportation by trained drivers in safe handicap accessible vehicles. Applicants will be accepted into the program based upon medical need and the recommendation of a physician. Eligibility will be determined based on physical and psychosocial impairment that require supervision or medical intervention or the need for rehabilitation services. Adult Day Care is a covered service under the New York State Medicaid program. Private payment is available for those not eligible for Medicaid.
