Details: Home Away from Home (ADHC) is a community-based program designed to meet the needs of adults with psychiatric, physical and/or cognitive impairments and chronic health conditions. Structured, comprehensive and non-residential, this program provides a variety of social, health, rehabilitative and related support services in a safe, culturally diverse setting. Our goal is to help prevent premature nursing home placement by offering programs and services for participants and respite for family.
