Our model of care is known as PACE (Program of All Inclusive Care for the Elderly). Participants attend the Providence ElderPlace Center on a regular basis, and transportation is provided. Polack Adult Day Center 7500 Seward Park Avenue South Seattle WA 98118 (206) 805-2337	The Polack Adult Day Center is located in the Seward Park lakeside campus of The Kline Galland Home. Operating Monday through Thursday, the Day Center provides socialization, recreational opportunities and health monitoring to older adults. Participants benefit from social interaction and stimulating activities in a supportive environment. Many times participants are able to remain in their homes longer with the support of our adult day programs. The Day Center also provides needed respite for caregivers and introduces participants and their families to the greater network of services of Kline Galland Center.
Elderwise Adult Day Center 900 University Street Seattle WA 98101 (206) 774-6606	Our Adult Day Program meets Monday through Friday from 10 a.m. until 2 p.m. at our home in the heart of Seattle near the intersection of the Downtown, First Hill and Capitol Hill neighborhoods. We are located inside Horizon House, the well-respected continuing care retirement community, and are pleased to serve residents of Horizon House as well as residents of the greater Seattle community. Our program uses the Elderwise enrichment, Spirit-centered Care model to provide a structured program of arts, exercise, discussion and shared community for the frail elder and respite for caregivers. Our program has a maximum of eight participants a day with 2 program facilitators â€“ creating an intimate social group of friends. Elderwise is not a drop-in program; participants always come on the same day. Thus someone who attends Tuesday will meet the same group of friends every Tuesday. Participants may attend from one day up to 5 days per week. The philosophy is inclusive, new people are always welcomed into the community.

Our goal is to help participants maintain independence, enhance their quality of life and promote health and wellness. If you have an elderly relative who would enjoy conversation over lunch with friends, benefit from mild physical and mental "exercise," and participate in outings in the community, Kokoro Kai is the program of choice for your loved one. Kokoro Kai, which means "a meeting of hearts and minds," is housed at Nikkei Manor, our assisted living facility in Seattle's International District.

Many of our clients are the pioneers who helped build the Chinatown-International District. Our seniors are usually first-generation immigrants who speak limited English and do not have family or friends to help them access traditional health care. Many have experienced feelings of loneliness and isolation. At Legacy House, elders with even the most limited incomes receive the supportive care they need to grow older with dignity. They are part of our supportive community that honors and reflects their cultural values. Our seniors are able to maintain a healthy, active and independent lifestyle as long as possible with the personal care or our dedicated and knowledgeable staff.
