Social model senior adult day program is a setting where seniors can gather for structured socialization and recreation in a supervised environment. A caregiver may choose adult day programs as a next step in the care of a loved one who may be in the early stages of dementia or declining in cognitive ability. Our programs allow caregivers to have much needed respite while assured that their loved one is in a monitored environment from one to five days a week. The Hebrew Home at Riverdale offers two unique senior adult day care programs serving the Bronx, Manhattan, East Side of Manhattan and Westchester County and medical facilities serving older adults with memory problems.

Our professional staff takes an interdisciplinary approach to develop an individualized plan of care for each client. We assist in the management of the many challenging situations faced by persons living with HIV/AIDS, coordinating, where appropriate, with other VillageCare AIDS services. Our centers have extended weekday hours and are open on weekends. We provide meals and MetroCards. Our services are for Medicaid-eligible persons. CARE, Center for Alzheimer's Respite Care for the Elderly, enhances the well-being and emotional health of mentally frail older adults, including persons with Alzheimer's disease or related dementia. CARE provides a safe, caring environment and helps older adults participate in group and individualized activities, while also providing a respite for full-time family caregivers. To enroll, clients must be 60 years of age or older, unable to function independently because of mental impairment and in need of support throughout the day. A diagnosis of dementia is required for participation in the program.

Our Adult Day Health Care program is a free-standing facility, providing comfort and promoting independence, through a full range of nursing, nutritional, case management, rehabilitative services and social activities. The Adult Day Health Care Center is for older adults who are able to live at home but may need some of the special services we offer. For example at home they may not have adequate supervision during the day which the Adult Day Health Care Center provides. Or perhaps they may have chronic medical problems or may need physical rehabilitation so living at home remains a safe option. All of the services that are available to residents are open to our Adult Day Health Care registrants, including medical care, physical and occupational therapies, and social events.
